The history of my illness goes deep into my childhood, but it manifested itself most of all six years ago, since then I have been suffering from depression, GAD with pan-attacks and phobias, occasional insomnia and nightmares due to PTSD (and a number of its symptoms).
But that's not the point. The bottom line is that, after a bunch of drugs prescribed to me, this one, so to speak, satisfied all my requirements. Depression is gradually fading away, working capacity has increased, a desire to create and live in general appeared, apathy subsided, I became calm, like a tank, for the first time in a long time I was finally able to sleep well with the lights off (yes, that was my main phobia, how would it didn't sound). Side effects came to light in the form that I took a beta-blocker for the prevention of pan. attacks, and I finally removed it, since Trittiko already has the property of an alpha-adrenergic blocker. Be careful with this! Low blood pressure, weakness, dizziness, palpitations and general discomfort in the region of the heart are clear reasons for reducing the dosage of an adrenergic blocker of any kind, or even stopping it altogether. Also be careful when using sedative drugs together, especially from a number of benzodiazepines (phenozepam, alprozalam, etc.). In principle, they are relatively easy to get off, thanks again to Trittiko.

Advantages- Fast action;
- It is effective both in the treatment of depression and in anxiety disorders, or even anxiety-depressive r-ve;
- Good and as a sleeping pill;
- Due to its adrenoceptor blocking effect, it is possible to reduce the severity of symptoms of pan-disorder

disadvantages- Not a very pleasant side effect in the form of menstrual irregularities, but I learned to live with it, because. I was told that with the long use of such drugs, this is also possible;
- Be careful when taking adrenoblockers at the same time.

I started taking 1/3 tablet, now it's full. Gradually increased the dose despite allergic side effects, tk. the drug itself improved the psychological state. But now I'm thinking of stopping taking it: constant inflammation of the eyes, itching on the face, dry mouth in the morning, but the most unpleasant thing is that an incomprehensible cough has been added. It's a pity.

A very good drug, if it doesn’t cause side effects in you, anxiety is forgotten, fears_ just gradually come to life! as always, oh, nothing will happen. A very good drug, if it doesn’t cause side effects in you, anxiety is forgotten, fears_ just gradually come to life! as always, ah, nothing will happen. But at night I got up, did not have time to reach ... It darkened in my eyes, grabbed a can of coffee. it’s back to normal - the pressure drops so quickly! Now I started with amitroptyline, it’s still better than others. I’m thinking of switching to tritico - I don’t know the doctor’s reaction.

I am 30 years old, since childhood I have been tormented by a constant feeling of anxiety, but still, that sometimes it’s impossible to do anything and the presence of fear, and depression came from this hopelessness, I tried many antidepressants, tranquilizers, but then the doctor prescribed me trittiko I drink 4 days 50mg and the effect is amazing, I feel... I had a bitter experience on the use of this Trittiko, got such side effects that you would not wish on the enemy. Although I drank according to the doctor's prescription. I took half a pill and it was still bad.

Trittiko I took last winter. I had a misfortune and a terrible depression began - I didn’t eat anything, while I smoked and drank coffee, I lost 5 kg in 2 weeks, I hardly slept at night, later I started having stomach pains, headache, nausea, black circles under my eyes . In general, bad condition. Mom sounded the alarm, at first she persuaded, then she tried to force at least something to eat or go for a walk, I didn’t even go outside, I lay in bed all the time and cried. Then she told me how she gave me Trittiko. She dissolved the pills for me in water. I followed the instructions strictly according to the instructions, and on the 5th day I noticed that the mood began to appear and somehow I wanted to eat. And my mother told me that she gave me this Trittiko. After using it, I came to life, it really helps.

A big request to actively add your reviews about the drug: did the medicine help or not help get rid of the disease, what complications and side effects were observed, perhaps not declared by the manufacturer in the annotation. Trittico analogues in the presence of existing structural analogues. Use to treat depression, anxiety, and phobias in adults, children, and pregnancy and lactation. The composition and interaction of the drug with alcohol.

Trittiko- an antidepressant. Trazodone (the active ingredient of Trittico), being a derivative of triazolopyridine, has a predominantly antidepressant effect, with some sedative and anxiolytic effects. Trazodone has no effect on MAO, which distinguishes it from MAO inhibitors, tricyclic antidepressants.

The mechanism of action of trazodone is associated with the high affinity of the drug for certain subtypes of serotonin receptors, with which trazodone enters into an antagonistic or agonistic interaction, depending on the subtype, as well as with the specific ability to cause inhibition of serotonin reuptake.

Quickly affects mental (affective tension, irritability, fear, insomnia) and somatic symptoms of anxiety (palpitations, headache, muscle pain, frequent urination, sweating, hyperventilation).

Trittiko is effective in sleep disorders in patients with depression, increases the depth and duration of sleep, restores its physiological structure and quality.

Trittiko stabilizes the emotional state, improves mood, reduces the pathological craving for alcohol in patients suffering from chronic alcoholism during the period of alcohol withdrawal syndrome, also in remission. In withdrawal symptoms in patients suffering from dependence on benzodiazepine derivatives, trazodone is effective in the treatment of anxiety-depressive conditions and sleep disorders. During remission, benzodiazepines can be completely replaced by trazodone.

Helps restore libido and potency in both depressed and non-depressed patients.

The drug is not addictive. The neutral uptake of norepinephrine and dopamine is little affected.

Does not affect body weight.

Composition

Trazodone hydrochloride + excipients.

Pharmacokinetics

Absorption of the drug from the gastrointestinal tract after oral administration is high. Taking trazodone during or immediately after a meal slows down the rate of absorption. Plasma protein binding - 89-95%. Penetrates through histohematic barriers, as well as into tissues and fluids (bile, saliva, breast milk). Trazodone is metabolized in the liver, the active metabolite is 1-m-chlorophenylpiperazine. Most of the metabolites are excreted by the kidneys - about 75%, and excretion is completely completed 98 hours after ingestion; about 20% is administered with bile.

Indications

  • anxiety-depressive states of endogenous nature (including involutional depressions);
  • psychogenic depressions (including reactive and neurotic depressions);
  • anxiety-depressive states against the background of organic diseases of the central nervous system (dementia, Alzheimer's disease, atherosclerosis of cerebral vessels);
  • depressive states with prolonged pain syndrome;
  • alcoholic depression;
  • benzodiazepine dependence;
  • libido and potency disorders (impotence), incl. in depressive states.

Release forms

75 mg and 150 mg controlled release tablets (long-acting).

Instructions for use and regimen

The drug is taken orally 30 minutes before a meal or 2-4 hours after a meal. The prescribed dose of the tablet should be taken whole, without chewing and drinking plenty of water.

Adults are prescribed in an initial dose of 100 mg 1 time per day after meals at bedtime. On the 4th day, you can increase the dose to 150 mg. In the future, in order to achieve the optimal therapeutic effect, the dose is increased by 50 mg per day every 3-4 days until the optimal dose is reached. A daily dose of more than 150 mg should be divided into 2 doses, with a smaller dose taken after dinner, and the main one at bedtime.

The maximum daily dose for outpatients is 450 mg.

The maximum daily dose for inpatients is 600 mg.

In the treatment of impotence, in the case of monotherapy with the drug, the recommended daily dose is more than 200 mg, with combined therapy - 50 mg.

In the treatment of benzodiazepine dependence, the recommended treatment regimen is based on a gradual, sometimes over several months, dose reduction of the benzodiazepine. Each time, reducing the dose of benzodiazepine by 1/4 or 1/2 tablets, add 50 mg of trazodone at the same time. This ratio is left unchanged for 3 weeks, then proceed to a further gradual reduction in the dose of benzodiazepines up to their complete abolition. Thereafter, reduce the daily dose of trazodone by 50 mg every 3 weeks.

Children aged 6-18 years: initial daily dose of 1.5-2 mg / kg per day, divided into several doses. If necessary, the dose is gradually increased to 6 mg / kg per day with an interval of 3-4 days.

Use in children

Contraindicated in children under 6 years of age. With caution, the drug should be prescribed to patients under the age of 18 years.

Use in elderly patients

For elderly and debilitated patients, the initial dose is up to 100 mg per day in divided doses or 1 time per day at bedtime. The dose may be increased under the supervision of a physician, depending on the effectiveness and tolerability of the drug. Usually no dose exceeding 300 mg per day is required.

special instructions

Since the drug has some adrenolytic activity, it is possible to develop bradycardia and lower blood pressure. Therefore, the drug should be used with caution in patients with impaired cardiac conduction, AV blockade of varying severity, patients with recent myocardial infarction.

When using Trittiko, a slight decrease in the number of leukocytes is possible, which does not require specific treatment, except in cases of severe leukopenia. Therefore, during the period of treatment, it is recommended to conduct studies of peripheral blood, especially in the presence of pain in the throat when swallowing and the appearance of fever.

The drug does not have an anticholinergic effect, therefore, it can be prescribed to elderly patients suffering from prostatic hypertrophy, angle-closure glaucoma, and cognitive impairment.

The drug is dispensed by prescription.

With the appearance of prolonged and inadequate erections, the patient should consult a doctor.

During the treatment period, the patient should refrain from drinking alcohol.

Influence on the ability to drive vehicles and control mechanisms

Since the drug has anxiolytic and sedative activity, it is possible to reduce the ability to concentrate and the speed of psychomotor reactions. During treatment, the patient should avoid engaging in potentially hazardous activities, incl. driving vehicles and operating mechanisms.

drug interaction

Trittico may enhance the effect of some antihypertensive drugs and usually requires a reduction in their doses.

Simultaneous administration with drugs that depress the central nervous system (including clonidine, methyldopa) enhances the effect of the latter.

Antihistamines and drugs with anticholinergic activity enhance the anticholinergic effect of trazodone.

Trazodone enhances and prolongs the sedative and anticholinergic effects of tricyclic antidepressants, haloperidol, loxapine, maprotiline, phenothiazine, pimozide, and thioxanthene.

MAO inhibitors increase the risk of side effects of trazodone.

When used together, it increases the concentration of digoxin and phenytoin in the blood plasma.

Tablets "Trittiko": instructions for use

Reviews indicate the need to study the instructions for the drug, since if used incorrectly, it can cause negative phenomena. According to the instructions, treatment begins with a daily dosage of 100 mg. From 1 to 3 days, the drug is taken once, at night. From the fourth day, if necessary, the dose is increased to 150 mg. In order to obtain the desired therapeutic effect, the dosage is increased gradually, once every 3-4 days. The daily rate of more than 150 mg is divided into two doses. A smaller dose is drunk at lunchtime, and the main amount of medication is taken before going to bed.

"Trittiko": analogues

So, your doctor has just prescribed you Trittiko.

Pain became a constant companion of many people. It interferes with normal life. For example, 21% of Austrians suffer from chronic pain. On average, it takes 7 years from the onset of pain to the appointment of adequate treatment.

Pain reduces physical activity and interferes with full communication. It affects sleep, mood, well-being. Most of all, it is sleep disorders that reduce the quality of life.

Dream lies on the border of the psyche and somatics (soma - body, Greek). Adequate sleep is very important for restoring strength - physical and mental.

Why are antidepressants prescribed for chronic pain?

Many people ask themselves the question, “Why should I take antidepressants if I am in pain but not depressed?”

The effect of pain is not limited to sensory (physical) sensations. Pain directly or indirectly affects the well-being of a person as a whole, changes his mood. The sensory component (feeling of pain) is inseparable from its influence on the emotional sphere.

Pain affects both behavior and well-being.

Medical science knows that antidepressants not only fight depression, anxiety, but also reduce pain. Therefore, antidepressants are included in the recommendations for the treatment of chronic pain.

Antidepressants interfere with the conduction of pain impulses in the spinal cord and brain and can suppress the foci of circulation of pain impulses. Antidepressants with a hypnotic effect, such as Trittiko, additionally eliminate sleep disturbances.

The effect of antidepressants in chronic pain does not occur immediately. On average, pain decreases after a few weeks from the start of taking the drug .. Whereas the improvement in the quality of sleep when taking Trittiko occurs already on the 1st day.

The time of taking the antidepressant is determined by the doctor; on average, it is 6 months. Further use is possible, but is associated with the risk of dependence.

Links of the same chain: pain - decreased activity - depression

Long-term bodily ailment is associated with a change in perception and thinking. Often changes occur imperceptibly for a person, but the inseparable physical and mental manifestations of pain need treatment, like many other diseases.

Pain affects many aspects of a person's daily life: activity, sleep, mood, initiative, well-being and, to the same extent, self-esteem.

A sparing attitude towards one's own body, reduced activity lead to a decrease in physical endurance and a painful reaction to stress. This, in turn, further reduces activity, and ultimately leads to social withdrawal, isolation, and depression.

More than 60% of people suffering from chronic pain complain of sleep disturbances. As a result, they not only notice an increase in pain, but in the future they develop depression and anxiety.

Vicious circle: pain and sleep disturbance

60-80% of people suffering from chronic pain have sleep disorders.

What refers to sleep disorders, from a medical point of view?

  1. Trouble falling asleep, waking up in the middle of the night, poor sleep quality.
  2. Mentioned violations that occur more than twice a week for a month or more.
  3. A significant lengthening of the time of wakefulness by reducing the time of sleep. Fatigue and weakness during the day, disrupting the usual activity.
  4. Lack of sleep or poor quality of sleep makes sleep a burden for such people.

The close relationship between sleep and pain sensations has been studied and explained as follows: the pathways along which pain signals are conducted pass near the sleep center in the brain, where they interact.

The signal to wake up occurs in the center of sleep even before the perception of the pain signal, and the person wakes up not from pain, but because of the occurrence of a false signal. If the pain and related sleep disturbances continue, there is a danger of their transition to a chronic form.

Vicious circle: sleep disturbance and pain

There is also an opposite relationship: increased pain due to sleep disturbance.

The sleep structure of a person suffering from chronic pain and a healthy person is significantly different: with pain, the deep sleep phase is shortened, which is important for restoring the body after the pain experienced during the day. The lack of recovery leads to a change in the pain threshold, and the next day the pain is perceived as more intense. The deep sleep phase is also important for skeletal muscle relaxation.

During the night, the deep sleep phase is replaced several times by REM sleep, at which point unwanted awakenings may occur.

The goal of effective therapy for sleep disorders is to restore its normal structure. This is how Trittiko works.

Trittiko:

  • Restores the structure of sleep
  • Reduces the intensity of pain
  • Relieves depression
  • anxieties
  • Eliminates mental disorders of sexual function

Trittiko has no effect on body weight.

Sometimes there are drowsiness, fatigue, at the very beginning of treatment - dizziness, nausea. Very rarely, cases of prolonged pathological erection in men (priapism) are described. In such a situation, you should seek medical attention from your doctor or the nearest hospital.

If you experience any side effects, tell your doctor or pharmacist at the pharmacy, carefully read the instructions for use of the drug.

To minimize potential side effects, start Trittico at a low dose. Try to take the drug once a day or leave most of the dose of the drug for the evening.

How do you yourself monitor the progress of pain management?

Never leave your doctor's office without answering all your questions. If you do not understand something, ask your doctor for clarification until everything becomes clear to you. In order not to forget what to ask, you can make a list of questions in advance at home.

Tell your doctor about your problems and worries.

Follow the treatment plan drawn up by your doctor. If you have new complaints, tell your doctor about it - he will adjust the treatment plan.

If the pain does not decrease or increases as well, tell your doctor. Tell him if you sleep well at night. Do not exaggerate or underestimate if you feel unwell. Remember that only you can feel when the pain subsides and sleep becomes complete.

Talk to family and friends about pain management to clear your own preconceptions.

Find out about your illness and treatment options, including psychotherapy (individually and in groups), therapeutic exercises and special exercises.

Try to be physically and socially active, even if it's hard for you.

Be patient – chronic disease management requires a lot of patience.

Be sure to tell those around you if you feel unwell.

Keep in touch, do not remove yourself from communicating with people!

  • in the presence of high sensitivity to the components included in the drug;
  • lactation period and pregnancy.
  • age less than 6 years.

Use with caution in persons with AV blockade, ventricular arrhythmia, myocardial infarction, arterial hypertension, hepatic or renal insufficiency, as well as persons under 18 years of age, due to the risk of developing suicidal behavior (aggression, suicidal plans, anger).

Appointment during pregnancy and lactation

There are no data on harm to the fetus, but if the expectant mother took Trittico before delivery, monitoring of the newborn is required. Take only under medical supervision and with caution.

Trazodone can penetrate into breast milk in small amounts, so it is recommended to interrupt breastfeeding during its use.

Dosage and method of application

As indicated in the instructions for use, Trittiko tablets are taken orally, swallowed whole, washed down with a sufficient amount of water, 0.5 hours before or 2-4 hours after a meal.

  • depression with and without anxiety: the initial dose is 100 mg once at bedtime. After three days of therapy, the dose can be increased to 150 mg. Further, until the optimal clinical effect is achieved, the dose should be increased by 50 mg every 3-4 days. If the daily dose exceeds 150 mg, then it should be divided into 2 doses: after dinner, a smaller dose is taken, before bedtime - the main one. The maximum daily dose: for outpatients - 450 mg, for patients in the hospital - 600 mg;
  • treatment of impotence: monotherapy - more than 200 mg per day, as part of combination therapy - 50 mg per day;
  • treatment of libido disorders: 50 mg per day;
  • benzodiazepine dependence: according to the scheme, which provides for a simultaneous decrease in the dose of benzodiazepine by 1/4 or 1/2 tablets and an increase in the dose of trazodone by 50 mg, take for 21 days. Then continue to gradually reduce the dose of benzodiazepines, each time increasing the dose of trazodone. After the complete withdrawal of benzodiazepine, the daily dose of trazodone is reduced by 50 mg with an interval of 21 days.

The initial dose of Trittiko for debilitated and elderly patients should be no more than 100 mg per day. It is selected individually and prescribed once at bedtime or divided into two doses. Increasing the dose should be carried out under the supervision of a physician, taking into account the effectiveness and tolerability of the drug. For this category of patients, it is not recommended to exceed the maximum daily dose of 300 mg.

For children aged 6 to 18 years, the dose is determined taking into account the weight of the child at the rate of 1.5-2 mg per 1 kg of body weight per day and divided into several doses. To achieve the desired therapeutic effect, the initial dose of Trittico can be gradually increased, observing an interval of 3-4 days. The maximum daily dose for children should not exceed 6 mg per 1 kg of body weight.

  • drowsiness;
  • dizziness;
  • vomit;
  • nausea;
  • respiratory failure;
  • convulsions.

In cases of severe overdose, a coma develops. Symptomatic complex from the side of the heart may include bradycardia and tachycardia. Symptoms may appear within a day after an overdose and later. Simultaneous use with other antidepressants is dangerous for the development of serotonin syndrome.

The drug has no specific antidote. Adults with an overdose of more than 1 g of trazodone, or children with an overdose of more than 0.15 g, should take activated charcoal within one hour of the onset of overdose symptoms. In other cases, gastric lavage is advisable.

It is necessary to monitor the patient's condition within twelve hours after an overdose. It is recommended to control blood pressure, pulse, blood oxygen saturation. In patients with heart disease, cardiac activity should be monitored.

For short-term convulsions, treatment is not indicated. Frequent and prolonged convulsions are eliminated by intravenous administration of Diazepam or Lorazepam. If these measures did not help to eliminate convulsions, put a dropper of Phenytoin.

With arterial hypotension and sedation, supportive and symptomatic therapy is used.

special instructions

Before you start using the drug, read the special instructions:

  1. Drinking alcohol while taking the medication is contraindicated.
  2. If, while taking the pills, the patient develops a prolonged and inadequate erection, it is necessary to consult a doctor.
  3. The lack of anticholinergic action in Trittiko allows its use in prostatic hypertrophy, angle-closure glaucoma, cognitive impairment, and elderly patients.
  4. When prescribing Trittiko to patients with impaired cardiac conduction, AV block or after a recent myocardial infarction, the adrenolytic activity of trazodone and the risk of developing bradycardia and lowering blood pressure should be taken into account.
  5. During the period of treatment, it is recommended to periodically conduct a peripheral blood test, including when pain in the throat occurs when swallowing or fever. This will allow timely diagnosis of agranulocytosis and establish cases of severe leukopenia.

During depression, the risk of suicidal thoughts or behavior increases, including against the background of taking antidepressants, especially under the age of 24 years. Therefore, before the onset of a pronounced remission, patients need careful monitoring. If you experience unusual changes in behavior, you should immediately seek advice from your doctor.

Interaction with other drugs

When using the drug, it is necessary to take into account the interaction with other drugs:

  1. A case of the development of arrhythmia of the "pirouette" type is described with the simultaneous use of trazodone with amiodarone.
  2. A case of a moderately pronounced decrease in the anticoagulant effect of warfarin with simultaneous use with trazodone is described.

  5. With simultaneous use with ethanol, the inhibitory effect on the central nervous system increases.
  6. With simultaneous use, an increase in the concentration of digoxin in the blood plasma and the development of symptoms of intoxication are possible.
  7. With simultaneous use with carbamazepine, a case of an increase in the concentration of carbamazepine in the blood plasma is described.
  8. With simultaneous use with thioridazine, an increase in the concentration of trazodone in the blood plasma is possible; with tryptophan - a case of development of anorexia, psychosis, hypomania is described; with phenytoin - a case of increased side effects of phenytoin is described; with fluoxetine - possibly increased side effects; with chlorpromazine, trifluoperazine - the development of arterial hypotension is possible.
